WHAT TO BRING FOR YOUR APPOINTMENT

  • Your Passport

  • 1 passport sized photo  (50mm x 70mm) for each applicant

  • Bring all Forms sent to you by the Immigration office. If you are applying for an UPFRONT medical or the NEW EXPRESS ENTRY MEDICAL, we will provide you with the necessary forms.

  • ALL previous medical documents, prescription lists

  • If you are applying for Express Entry please bring your express entry number to the appointment.

  • If you are applying under the sponsorship category, please bring your medical form with the IME number.

  • Refugee applicants must bring all their immigration documents, including Interim Federal Health Form as well as a passport sized photo for each applicant.

  • Payment in Cash, Etransfer, Cashier Cheque.

Before leaving our clinic you will be given a copy of your information sheet with your case number and picture on it, and this will enable you to track your progress through the system on the Immigration, Refugees and Citizenship Canada (IRCC) website.  

After the exam is completed, please proceed to the xray clinic  (if over 11 years of age), and blood work clinic across the street.

 

Most medical reports are submitted between 5 and 10 days of completing any required examinations.
