WHAT YOU NEED:

Your Passport

Any Forms sent to you by the Immigration office. If you are applying for an UPFRONT medical or the NEW EXPRESS ENTRY MEDICAL, we will provide you with the necessary forms.

ALL previous medical documents, prescription lists

If you are applying for Express Entry please bring your express entry number to the appointment.

If you are applying under the sponsorship category, please bring your medical form with the IME number.

Refugee applicants must bring all their immigration documents, including Interim Federal Health Form as well as a passport sized photo for each applicant.
You will be given a copy of your information sheet with your case number and picture on it, and this will enable you to track your progress through the system on the Immigration, Refugees and Citizenship Canada (IRCC) website.
COMMON QUESTIONS:
X-RAY LOCATION
X-Rays are completed at KMI X-Ray and Ultrasound at 835 Princess Street, K7L 1G7.
BLOOD WORK LOCATION
Blood work is completed at Life Labs at 791 Princess St #102, K7L 1E9.
You can actually complete this part at ANY lifelabs location in Ontario. The results will be faxed back to us automatically.
